    Creating & Customizing Lead Forms

    Lead forms are how prospects enter your pipeline. GymPoint lets you create branded, customizable forms that you can embed on your website or share as a standalone link. Every submission automatically creates a new lead in your pipeline.

    Creating a New Form

    1. Navigate to Members > Memberships > Forms from the left sidebar.
    2. Click + New Form in the top-right corner.
    3. Enter a Form Name (e.g., "Website Inquiry," "Free Trial Request," or "Summer Promo Sign-Up"). This name is for your internal reference and will not be shown to visitors.
    4. Choose the Pipeline Stage where new submissions should land. Most gyms use the default "New" stage.
    5. Optionally, set a Default Lead Owner to auto-assign incoming leads from this specific form.
    6. Click Create to generate your form.

    You will be taken to the form builder where you can customize fields and appearance.

    Adding and Removing Fields

    The form builder lets you control exactly what information you collect:

    1. In the form editor, you will see default fields: First Name, Last Name, Email, and Phone.
    2. To add a field, click + Add Field and choose from available field types:
      • Text — Short free-text input (e.g., "How did you hear about us?")
      • Dropdown — Predefined choices (e.g., membership interest level)
      • Checkbox — Yes/no options (e.g., "I agree to be contacted")
      • Date — Date picker (e.g., preferred start date)
      • Paragraph — Longer text input (e.g., fitness goals)
    3. To remove a field, hover over it and click the trash icon. Note that First Name and Email are required and cannot be removed.
    4. To reorder fields, drag them into your preferred sequence.
    5. Mark fields as Required by toggling the switch next to each field name.
    6. Click Save when you are finished editing.

    Customizing Form Appearance

    Under the Style tab in the form editor:

    • Upload your gym logo to display at the top of the form.
    • Set your brand color for the submit button and accent elements.
    • Edit the submit button text (e.g., "Get My Free Trial" instead of the default "Submit").
    • Add a thank-you message that displays after successful submission.

    Embedding on Your Website

    GymPoint provides two ways to add your form to your website:

    Embed Code (iframe)

    1. In the form editor, click the Embed tab.
    2. Copy the provided HTML snippet. It will look similar to:

    3. Paste this code into your website's HTML wherever you want the form to appear.
    4. Adjust the height value if the form is taller or shorter than the default.

    Public Form URL

    Every form also has a standalone public URL that you can share directly:

    1. In the Embed tab, find the Public URL field.
    2. Copy the link (e.g., https://app.gympoint.ai/book/form/your-form-id).
    3. Share this link in social media posts, email campaigns, QR codes, or anywhere you want to capture leads.

    The public URL opens a full-page, mobile-friendly version of your form.

    Testing Your Form

    Before going live, always submit a test entry:

    1. Open your form using the Public URL or preview button.
    2. Fill in sample data and click submit.
    3. Check your Marketing > Pipeline to confirm the test lead appeared in the correct stage.
    4. Delete the test lead when you are satisfied everything is working.
